Medical Questionnaire
With professional scuba diving instructors, well-maintained quality dive gear and good health, scuba diving is safe.

At Total Diving, we specialize in scuba diving but we're not doctors! It's up to you to determine if your personal health condition is OK for scuba diving. To help you, PADI and the RSTC have prepared a self-assessment medical questionnaire for scuba diving.
If you have one of the conditions listed on that form (ie, if you answer yes at one of these items), we will need this form to be signed by your doctor before we can jump in the water with you.
Answering "yes" to one of these questions doesn't necessarily mean that you cannot dive. Only your doctor can determine that.
That's no big deal, is it? I bet you get your car checked and serviced twice a year. So... How about getting your own body checked at least once in a blue moon?!
It is best to consult your regular family doctor because he knows your health history. If this is not possible, we suggest Dr. Peter Vaktor who is a PADI divemaster and a doctor specialized in hyperbaric and scuba diving medecine. He is located downtown Montreal. When calling for an appointment, mention that it is for a scuba diving course at Total Diving then go see him, with the medical questionnaire.

For instance, you must answer each question with a YES or a NO. Nothing else is acceptable. For instance, writing Y or N is not OK. Sorry!
